*After completing and submitting the form, you will receive an email in which you must confirm the submission of your application so that the registrar’s office can receive it.
The Registrar’s Office responds to the Vice President for Student Affairs. The primary role of the Registrar is to maintain the permanent records and documents that pertain to each enrolled student’s progress through medical or graduate school following the regulations of the Buckley Amendment (Buckley Law).
Our main goal is to provide excellent services to all students, administration, and faculty.
- Coordinating Registration Enrollment Procedures.
- Offers orientation for Veterans Services and certifications.
- Provide certifications of Enrollment as required.
- Prepare the evaluation of student academic progress and graduation procedures, including diplomas.
- Certify Degrees and Academic Standing Letter.
- Certify Enrollment documents for USMLE Part I and II.
- Prepare, maintain, and distribute the Official Transcript.
- Comply with established policies for accessing student academic records based on the Federal Ed Rights and Privacy Act (FERPA).
- Orientation of the Institutional Health Insurance.
- Among other services.
Transcripts
A Transcript is an official document of the student’s academic record; copies are made upon the student’s request.
Our students can visit the National Student Clearinghouse portal for all transcript requests. Ponce Health Sciences University and the National Student Clearinghouse provide our students with a safe and reliable tool where you can request your Transcript online.
- Official transcripts must bear the school seal and will be sent directly to the institution as requested by the student. They will not be given to the applicant or a third person under any circumstance.
- Transcripts requested for personal use can be handed to the student personally; if the student authorizes another person to come for the transcript, written authorization is needed, and the person must have identification. Transcripts handed to the student would be labeled as a “student copy.”
- No transcripts will be honored if the student has outstanding debts.
- Requests will be processed in the order they are received in 5-7 business days.
- Transcript Fees are $15.00 per copy and $2.90 for the National Student Clearinghouse Service Fee per order.
Certifications
ORDERING
To order a certification, students must complete the Certification Request Form (link) and pay the fee to the Bursar’s Office. The Registrar’s Office can provide a certification based on the needs of our students. Requests will be processed in the order they are received in 5-7 business days.
PAYMENT
Each Certification copy is US $15.00.
Payments can be made through phone calls using VISA® or Master Cards®.
